POSITION SUMMARY
The MAT Case Manager at NORCOR plays a critical role in providing medical support and case management services to incarcerated individuals within the jail and juvenile detention facility. This position ensures the proper management of healthcare needs, including assessments, care coordination, and treatment planning for both adults and juveniles in the facility. The MAT Case Manager will work closely with correctional officers, healthcare professionals, and external providers to support the well-being of individuals in custody.
DUTIES AND RESPONSIBILITIES
The duties and responsibilities outlined below describe a typical work week. This list is not exhaustive and may contain additional functions of the position, as required to conduct day-to-day business.
Assessments & Care Planning:
- Ensure a health assessment is completed by a licensed nurse for each individual, to determine current medical needs and assess health history.
- Collaborate with community case manages to develop or enhance individualized care plans based on medical, mental health, and substance abuse histories.
- Collaborate with healthcare providers, mental health professionals, and social services to ensure comprehensive care for each individual.
Case Management:
- Monitor and coordinate medical treatments.
- Facilitate ongoing medical care and review the progress of treatment plans.
- Ensure the timely and accurate transfer of medical records and care plans between the facility and external healthcare providers when necessary.
Health Education & Advocacy:
- Provide health education and support to detainees on managing chronic conditions, medications, and healthy lifestyles.
- Advocate for detainees to receive necessary medical, psychiatric, and dental care while in custody.
Collaboration with Facility Staff:
- Work alongside detention facility staff, including officers and social workers, to ensure a safe and supportive environment for medical care.
- Assist in addressing any medical emergencies or concerns within the facility.
Documentation & Compliance:
- Maintain accurate and up-to-date medical records and case files in accordance with facility policies and regulations.
- Ensure compliance with healthcare-related laws, regulations, and ethical standards.
Crisis Intervention:
- Provide support and de-escalation in situations involving medical or mental health crises.
- Serve as a resource for facility staff in addressing medical emergencies or behavioral health concerns.
Re-Entry
- Communicate and coordinate with Coordinated Care Organization and health (medical and behavioral) care providers as individual’s transition into next placement or community.
QUALIFICATIONS
Education & Certification:
- Bachelor’s degree in Nursing, Social Work, Health Administration, or a related field. (preferred)
- Current certification as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), or Social Work licensure, is highly preferred.
- Certification in case management (e.g., CCM, CMCN) is a plus.
Experience:
- Minimum of 2 years of experience in case management, healthcare, or a related field, preferably in a correctional or juvenile detention setting.
- Experience working with diverse populations, including juveniles, individuals with substance abuse issues, and those with chronic medical conditions.
Skills:
- Strong communication, interpersonal, and organizational skills.
- Ability to assess medical and behavioral health needs and develop appropriate care plans.
- Knowledge of healthcare policies, regulations, and ethical standards.
- Ability to work in a high-stress, secure environment and manage multiple priorities.
- Cultural competence and sensitivity to working with diverse populations.
PHYSICAL DEMANDS:

Work is generally performed indoors in a detention environment and is essentially sedentary in nature. The work requirements include the ability to see, talk and hear, sit and stand, walk, use hand to finger, handle or operate objects, tools or controls; reach with hands and arms and lift or move (50) fifty pounds. Ability to remain on foot for extended periods. Capability to lift up to 25 pounds. Ability to respond quickly in emergencies and assist detainees in a physically demanding environment.
Some work assignments directly involve the safety and security of the facility and may require split second decision making.
WORK ENVIRONMENT
Detention facilities are unique by nature. Communications may be by electronic means, requiring visual and audio queues and codes. The noise level in the workplace is usually moderate but is unpredictable and can escalate to a loud level of abusive verbal sounds and terminology. There may also be occasional occurrences of speaker systems, percussive and abrasive sounds, alarms and recreational sounds.
Working conditions are generally safe and situations may arise where the safety of the employee could be compromised, including verbal abuse, threats, physical assault, etc. While every effort is made to ensure employee safety through training and the provision of personal protective equipment (PPE), the employee may be exposed to contagious, or infectious organisms, bacteria, viruses, and other biohazardous materials.
