Position: Home Health Nurse (RN/LPN)

- Provide skilled nursing care for an infant or pediatric patient with complex medical needs.
- Manage ventilator, trach, and J-tube equipment and treatments.
- Monitor the patient’s health and respond to changes in condition.
- Collaborate with family members and interdisciplinary teams to ensure optimal care.

Position: Home Health Nurse (RN/LPN) - Nights
Location: Lakland and surrounding areas
Shift: Full-time & Part-time, 12-hour shifts
(7pm to 7am or 8p-8a)-every other weekend
North Bay Home Health is a provider of in-home care services specially designed to ensure the needs of our pediatric patients (ranging from infants to young adults) are a priority. We provide assistance to parents/guardians with daily caregiving needs. We focus on ensuring care that mirrors the request set forward by the Pediatrician and the clients’ Primary Care Provider. NBHH offers an array of home-based services that assist individuals with full recover or the ability to manage their condition all in the comfort of their own home.
Responsibilities:
- Deliver high-quality, patient-centered care to individuals in their homes, adhering to physician orders and personalized care plans.
- Conduct thorough assessments and develop tailored nursing interventions to address patients' needs.
- Administer medications, treatments, and procedures in accordance with established protocols and regulatory standards.
- Monitor patients' progress and collaborate with the interdisciplinary team to adjust care plans as needed.
- Educate patients and their families on proper disease management, medication administration, and self-care techniques.
License Requirements:
- Current and active FL or Multistate RN/LPN license (Required)
- BLS CPR card (Required) via AHA or ARC
Experience:
- Pediatric experience (STRONGLY Preferred)
- G-tube, Trach, and Vent experience (MUST)
- At least one year of home health experience
- Experience with developmentally delayed patients
Schedule:
Shift: Full-time & Part-time, 12-hour shifts NIGHTS
Pay Rate
· LPN $23-$29/hr. RN $29-$34/hr.
