Position: Home Health Nurse (RN/LPN)

Key Responsibilities:
- Deliver specialized nursing care to an infant or pediatric patient with complex medical needs.
- Oversee and manage ventilator, tracheostomy, and J-tube care and treatments.
- Closely monitor the patient’s condition and respond effectively to any health changes.
- Work collaboratively with family members and healthcare teams to ensure the highest quality of care.

Shift: Full-time & Part-time, 12-hour shifts
(7pm to 7am)-every other weekend
North Bay Home Health is a provider of in-home care services specially designed to ensure the needs of our pediatric patients (ranging from infants to young adults) are a priority. We provide assistance to parents/guardians with daily caregiving needs. We focus on ensuring care that mirrors the request set forward by the Pediatrician and the clients’ Primary Care Provider. NBHH offers an array of home-based services that assist individuals with full recover or the ability to manage their condition all in the comfort of their own home.
Responsibilities:
- Deliver high-quality, patient-centered care to individuals in their homes, adhering to physician orders and personalized care plans.
- Conduct thorough assessments and develop tailored nursing interventions to address patients' needs.
- Administer medications, treatments, and procedures in accordance with established protocols and regulatory standards.
- Monitor patients' progress and collaborate with the interdisciplinary team to adjust care plans as needed.
- Educate patients and their families on proper disease management, medication administration, and self-care techniques.
License Requirements:
- Current and active FL or Multistate RN/LPN license (Required)
- BLS CPR card (Required) via AHA or ARC
Experience:
- (STRONGLY Preferred)
- G-tube, Trach, and Vent experience (MUST)
- At least one year of home health experience
- Experience with developmentally delayed patients
Schedule:
Shift: Full-time & Part-time, 12-hour shifts NIGHTS
Pay Rate
· LPN $23-$29/hr. RN $29-$34/hr.
