Job Description:
We are seeking a dedicated Nurse Practitioner (NP) to join our primary care team. The ideal candidate will provide comprehensive medical care, including diagnosing and managing acute and chronic conditions, performing wellness exams, and developing treatment plans tailored to individual patient needs.
Responsibilities:
- Conduct physical exams, diagnose conditions, and develop treatment plans
- Manage chronic diseases such as diabetes, hypertension, and asthma
- Order and interpret diagnostic tests
- Prescribe medications and educate patients on treatment options
- Provide preventive care, including immunizations and health screenings
- Collaborate with physicians, nurses, and other healthcare professionals
- Maintain accurate and up-to-date patient records in the EMR system
- Educate patients on lifestyle modifications and disease prevention
Qualifications:
- Education: Master’s or Doctorate degree in Nursing (MSN/DNP)
- Licensure: Current Nurse Practitioner license in New York
- Certification: Board certification (ANCC or AANP) in Family, Adult, or Experience in Primary care of experience in a primary care or outpatient setting preferred
- Skills: Strong clinical assessment and diagnostic skills, excellent communication and patient-centered approach
