SUMMARY OF RESPONSIBILITIES:
Will assist individual's who have a serious mental illness (adults), serious emotional disturbance (children) or intellectual/developmental disability (adults/children) in accessing needed medical, social, educational and other services in order to remain as independent as possible as identified through Person Centered Planning. Core elements of case management are assessment, service plan development, linking/coordination of services, reassessment/follow-up, and monitoring of services. May be responsible for the initial assessment of persons requesting services from Northeast Michigan Community Mental Health Authority and appropriate referral to internal or external providers or community resources.

Rate of Pay: $58,552 - $80,891.2

FLSA: Exempt/Full Time

Reports To: Program Manager or Designee

QUALIFICATIONS:
Education: Masters degree from a recognized or accredited institution, with course work in mental health related areas. Must attain highest level of licensure according to state guidelines.
Experience: One or more years of relevant mental health experience. Lived experience with mental illness, intellectual/developmental disabilities, and/or substance use disorders valued.
Other: Good communication skills (both verbal and written) are required. Functional computer literacy is required. Must have Michigan driver's license and a satisfactory driving record as determined by the
agency.

ESSENTIAL JOB FUNCTIONS:
1. May provide individual or group therapy.
2. Will provide assessment, service plan development, linking/coordination of services, reassessment/follow-up, and monitoring of services.
3. Participates as an active member of the Children's Diagnostic and Treatment Team.
4. Demonstrates accurate assessment and diagnosing for persons requesting Mental Health Services in accordance to the latest edition of the DSM and ICD.
5. Demonstrate effective case recording in accordance with Person Centered Planning, and NeMCMHA "Case Recording Guidelines".
6. Provide clinical assessments, crisis intervention, and participate in the agency's emergency services on-call rotation.
7. Collaborate and consults with agency staff, various agencies, educational settings and organizations as needed.
Staff will be expected to participate in trainings aimed at the provision of services for individual's with cooccurring disorders.
8. Demonstrates knowledge of universal precautions, standards of practice including but not limited to confidentiality and recipient rights, and emergency protocol.

NON-ESSENTIAL JOB FUNCTIONS:
1. Performs other related duties as required.
2. May be requested to be involved in the Board's continuous quality improvement program.
3. May be required to provide own transportation while performing agency-related duties.

The physical demands here are representative of those that must be met by an employee to successfully perform the functions of this job. Ability to walk, bend, stand, sit, lift up to 25 pounds, with or without assistance, stretch/reach, hear, see, hand/finger dexterity and drive.
