POSITION TITLE: Fatherhood Program Case Manager
REPORTS TO: Fatherhood Program Director
FSLA CLASSIFICATION: Non-exempt
Rate: $21.22
GENERAL RESPONSIBILITY:
The Fatherhood Program Case Manager will assist fathers with achieving employment and education goals and will incorporate aspects of conflict management, violence prevention, and other support services to help fathers resolve issues and achieve economic security.
SPECIFIC RESPONSIBILITIES:
- Assist with recruitment of program participants.
- Complete eligibility screening, intake, and comprehensive needs assessments with each client.
- Use the above results to complete Individualized Development Plans (IDP) for each participant.
- Schedule at least eight case management sessions with each client.
- Monitor and document IDP progress, barriers, and changes.
- Provide comprehensive referrals and links to services.
- Meet with program participants individually and in groups.
- Complete Program orientation with participants.
- Facilitate the Fatherhood Program, including facilitating the fatherhood curriculum (some weekends and evenings required)
- Track attendance/participation levels and follow up with participants who begin to disengage to encourage their continued participation.
- Provide at least 3 months of follow-up support after participants complete or exit the program.
- Identify and address challenges for graduates during follow-up period.
- Work with service providers to establish/maintain resources linkages.
- Assist Fatherhood Program case management team members in facilitating job/internship placements and service linkages.
- Document and report on follow-up services.
- Participate in CQI and program evaluation.
- Attending all required trainings as deemed necessary.
- Performing other duties relevant to the agency, as assigned.
Qualifications
QUALIFICATIONS AND EXPERIENCE:
- Associate degree in human services or related field.
- Experience in case management.
- Must possess excellent interpersonal skills.
- Must possess excellent written and oral communication skills.
- Ability to relate to persons of various socio-economic backgrounds.
- Reliable transportation.
PHYSICAL REQUIREMENTS:
- Ability to bend, lift up to 50 pounds and engage in a full range of motion.
- Ability to sit, read, and type at a computer for up to 8 hours per day.
