Responsibilities
This Director level pharmacist will play a critical role in integrating population health strategies into pharmacy practices to improve patient outcomes, reduce healthcare costs, and enhance the quality of care across diverse patient populations. Unique and core responsibilities for this pharmacy director are strategic direction, financial/budgetary/contractual management and operational oversight for the NHC employee prescription benefit plan (>$110,000,000/yr paid claims, >400,000/yr script volume and >30,000 covered members) and other NHC population-based medication initiatives. This pharmacist will demonstrate the value of population health medication management using contemporary population health management models, data analytics and outcomes measurements. This pharmacist will conceptualize and manage comprehensive medication therapy management services aligned with population health goals through innovating and growing clinical programming. This pharmacist will possess expertise in evolving payer models, contracting, risk management, regulations and compliance. This pharmacist will be committed to achieving and maintaining population health medication management in accordance with the pharmacy department and Norton Healthcare strategic direction.
Key Accountabilities:
- Provide strategic, financial/budget, contractual and operational oversight for the NHC employee prescription benefit plan and other NHC population-based medication initiatives. Utilizes data analytics and outcomes measurements to demonstrate value of population health medication management.
- Develops education and workforce to achieve population health management
- Demonstrate expertise in health system pharmacy practice including awareness of evolving payer models, contracting, risk management, regulations and compliance. Engage with pharmacy executives and peer leaders serving as a subject matter expert in population health management.
- Conceptualize and manage medication therapy management services aligned with population health goals that deliver improved patient outcomes and net lowest costs. Implement processes that drive quality improvement through reduction of health disparities and enhanced medication safety.
Qualifications
Required:
- Three years of work experience in population health management or similar work.
- Bachelor Degree in Pharmacy
