About the Role
Ciena Healthcare is seeking a highly skilled Medical Case Manager Coordinator to join our team. As an MDS Coordinator, you will provide oversight of the Resident Assessment Instrument (RAI) process and conduct assessments and care plan coordination for our guests.
The ideal candidate will have experience as an MDS Nurse and possess knowledge of the RAI process, including the principles of the Prospective Payment Process (PPS). AANC certification and RAC-CT are desirable but not required.
Key Responsibilities:
* Completes the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care plans within regulated time frames.
* Coordinates scheduling the RAI process with the interdisciplinary team.
* Assesses residents through physical assessment, interview, and chart review.
* Discusses resident care needs with caregivers, including physicians, nurses, social services, therapy, dietary, and activity staff.
* Reviews information from hospitals, consults, and outside agencies and uses such information in the completion of the assessment and care planning.
* Coordinates, identifies, and/or initiates significant change in MDS'
* Prepares for PPS meetings, maintaining MDS assessments per Medicare schedule, and monitors Medicare days and RUGs utilization in the absence of the Care Management Coordinator.
