- Qualifications :
- REQUIREMENTS :
- Required : Doctor of Pharmacy required from an accredited institution.
- Required : Completion of PGY-1 residency in an acute care setting with additional 2 (two) years of clinical experience or completion of PGY-2 residency.
- Preferred : 2 years Residency training.
- Required : Licensed to practice in the State of Texas.
- Required : Completion of an accredited Residency in Hospital Pharmacy, specialty training in Disease State Management.
- Required : ACPE IV certification.
- Preferred : Antimicrobial stewardship certification.
- Required : ACLS certification.
- Required : AHA BLS.
- Encouraged any Board of Pharmacy Specialty certifications.
