Responsibilities
- Provides community-based services to consumers and consumer’s family in the community;
- Facilitates service linkage in the mental health and non-mental health, health system; coordinating and integrating services from multiple providers;
- Assists and supports individuals in developing skills to gain access to needed medical, behavioral health, housing, employment, social, educational and other services essential to meeting basic needs;
- Provides training and/or direct service support to individuals in the use of basic community resources such as transportation and financial resources;
- Provides necessary support to ensure that individuals needs and goals are addressed, including aggressive advocacy, escort to appointment, daily living skill remediation and money management;
- Provides accurate and timely documentation of services in the electronic health record;
- Provides 24 hours/7 days per week on call crises intervention, including in person response on a rotating basis, including evenings and weekends;
- Works collaboratively with the Clinical Team;

- Bachelor’s degree in a related field
- 1 year experience working in a behavioral health setting preferred;
- Valid Driver’s License
