As a Case Manager, you will work within our Crisis Diversion program located in Trenton, Mercer County, NJ.
Schedule: Full-time; Monday-Friday 9:00am-5:30pm; Saturday: Rotation emergency on-call; Half hour unpaid lunch
Competencies:
The potential employee will have the ability assess and interpret information related to population; have knowledge of the symptoms of the disability/disorder and ability to recognize exacerbation of symptoms; have a general understanding of treatment needs; knowledge of available treatment resources and appropriate use; Population-specific communication skills.
Responsibilities:
- Maintains thorough knowledge of each consumer enrolled in Crisis Diversion and shares this knowledge with other team members through
- Active participation in team meetings;
- Regular review of each consumer's case record to ensure optimal responsiveness by all team members to clients' changing needs; consulting with other staff regarding client's progress; regular meetings with consumers in accordance with the treatment plan;
- Regular progress updates to all team members concerning work performed on consumer's behalf - recording same in the case record within 24 hours - to ensure the timely sharing of vital information about each consumer;
- Other appropriate means of information sharing to ensure speedy and knowledgeable response to service provision to Crisis Diversion consumers
- Provides crisis intervention and stabilization services for clients referred by the Screening Center
- Provides intensive case management services for 2 months.
- Runs partial care groups for Crisis Diversion clients
- Provides supportive services to clients including transportation as appropriate.
- Facilitates linkage of clients to mental health treatment, substance abuse treatment, housing, and other social service resources
- Shares in "after-hours" duties that ensure 24-hour, 7-day coverage for ICM consumers, including after hours on-call rotation, after-hours outreach to ICM consumers, and after-hours crisis intervention coordinated with emergency/screening service providers, among other after-hours duties as may be identified by the ICM Team.
- Maintains client records and documentation of services.
- Participates in staff meetings and other program activities.
- Makes recommendations to supervisor regarding program needs, ideas, and areas for improvement.

Qualifications:
- Master’s degree (MA/MS) in social work or related field, OR;
- Bachelor’s degree (BA/BS) in social work or related field with two (2) year’s related experience with mental health;
- Valid Driver's License in good standing required.
