DESCRIPTION

The Onondaga County Health Department is seeking a qualified candidate to support the Mental Health and Substance Use Initiatives Program in Onondaga County. This position will work with community members with substance use disorders to improve their access to community services and the healthcare systems.

The Service Navigator (Health) will work collaboratively within the community to develop rapport with clients and foster relationships with other service providers. The Service Coordinator will regularly engage in community outreach and will cooperate with the entire Substance Use Initiatives team to provide comprehensive, empathetic care. The position reports to the Program Coordinator of Substance Use Initiatives.

· Contact potential participants and referrals within one business day.

· Conduct street and community outreach to individuals who might not otherwise seek assistance for issues related to substance use.

· Connect individuals and families to identified substance-use supports and services.

· Acts as a referral agent for patients to other Health Department and community-based programs and services

· Assists participants in understanding and accessing available community supports and resources

· Collaborates with care managers and patient navigators from community agencies

· Maintains contact with clients, including phone contacts and field visits

· Conduct outreach efforts to participants who have been disengaged from services.

· Completes required documentation, including tracking mechanisms progress notes and care plans in accordance with agency standards.

· Attend and participate in monthly staff meetings.

MINIMUM QUALIFICATIONS

A. Graduation from a regionally accredited college or university or one accredited by the New York State Board of Regents to grant degrees with a Bachelor’s degree in any field and two (2) years of experience, or its part-time equivalent, as a professional advocate or service provider in the field of rehabilitation, substance use disorder recovery, substance use disorder treatment or mental health; or,

B. Graduation from a regionally accredited college or university or one accredited by the New York State Board of Regents to grant degrees with an Associate’s degree in any field and four (4) years of experience, or its part-time equivalent, as a professional advocate or service provider in the field of rehabilitation, substance use disorder recovery, substance use disorder treatment or mental health; or,

C. Graduation with a high school diploma or holder of a high school equivalency diploma issued by an education department of any of the states of the United States or holders of a comparable diploma issued by any territory, commonwealth, or possession of the United States or by the Canal Zone or holders of a report from the United States Armed Forces certifying successful completion of the tests related to general education development, high school level and six (6) years of experience, or its part-time equivalent, as a professional advocate or service provider in the field of rehabilitation, substance use disorder recovery, substance use disorder treatment or mental health; or,

Completion of a Master’s degree in health education, public health, health promotion, nursing, public health administration, public administration or a closely related health field may be substituted for one (1) year of work experience.

Desired Experience:

· Ability to assist individuals and families in navigating the Substance Use Services system.

· Ability to assess needs and then develop a person-centered plan with the individual.

· Comprehensive knowledge of local agencies and Health Department services

· Ability to establish effective working relationships with health professionals, program participants and clients

· Ability to organize and collaborate with other services to deliver person-centered services

· Ability to follow oral and written instructions

· Ability to understand and interpret written information

· Ability to coordinate an individual’s care and collaboration with other services.

· Ability to teach life skills as needed. Experience working with people who use drugs, and individuals with mental health conditions

Other Requirements:

· Must have a valid New York State Driver’s License.

· Be able to sit or stand as needed, with or without reasonable accommodation.

· Be able to walk, primarily on a level service, for sometimes extended periods throughout the day, with or without reasonable accommodation.

· Be able to reach above shoulder height, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation.

PROVISIONAL APPOINTMENT

Successful candidate will be appointed provisionally and must take the corresponding Civil Service exam, scheduled for a later date, and place high enough on the list to qualify for permanent appointment.

Position Schedule: 35 hours per week, Monday – Friday, 8:30am – 4:30pm. Candidates must have the ability to work occasional flexible hours, which may include evenings and weekends.
