On-Call Nurse/Nurse Supervisor
Position Overview: The On-Call Nurse/Nurse Supervisor is available during agency on-call hours to provide and direct nursing care and implement the medical plan of treatment that is prescribed by a licensed physician. The scope of the nursing practice will include assessing, diagnosing, and treating the human response to actual or potential physical or emotional health problems (ANA Nursing Practice). This practice will be implemented through preventative, supportive, and restorative care. This care is delivered within the guidelines of the ANA Standards of Care, and the policies of Open Systems Healthcare, along with state, federal, and local laws/regulations.
Essential Job Functions and Performance Responsibilities:
- Maintains standards of nursing care and implements the policies and proceduresestablished by the Agency.
- Maintains and adheres to Agency procedure and client care policy manuals.
- Follows policies and procedures that are consistent with recommended Centers of DiseaseControl (CDC) and Occupational Safety and Health Agency (OSHA) guidelines for safety,universal precautions, and infection control procedures.
- Follows the Tuberculosis screening program and Hepatitis B program, if applicable.
- Competently assess and document the client's biophysical, psychosocial, and educationalneeds.
- Interprets to the client and family the implications of the client’s health status.
- Demonstrates the ability to communicate effectively with other members of the health careteam and staff of the Agency.
- Consistently reports appropriate changes in the client’s condition to the DON and thephysician.
- Ensures accuracy, completeness, and timeliness of documentation in accordance withAgency policies and procedures, regulatory requirements, and industry standards.
- Administers and documents the client’s prescribed medication competently.
- Demonstrates competence in performing all treatments per Physician’s Plan of Care.
- Consistently adheres to universal precautions, aseptic technique, and infection controlguidelines.
- Consistently implements care in a manner that is safe for the client, his/her family, and self.
- Consistently seeks, accepts, and implements suggestions to improve performance.
- Demonstrates respect for the opinions of others.
- Consistently assumes and follows through on the responsibilities of the assignment.
- Demonstrates the ability to function effectively under stressful situations.
- Performs 60 day re-assessments and post-fall assessments, as needed.
Required Qualifications:
- Graduate of an accredited School of Nursing required.
- Current and valid RN license in the state of NJ.
- Evidence of Tuberculosis screening that meets CDC guidelines.
- Evidence of passed Home Care Exam.
- Availability via telephone and to provide home visits, if necessary, during Agency On-CallHours.
- Professional Liability Insurance with 1-3 million dollars in coverage, if applicable.
- A valid driver’s license.
- A valid auto insurance policy with liability coverage.
- Current and valid CPR certification.
- Current State mandates CEUs.
Experience:
- One year of professional experience as a Registered Nurse
- Experience in home care nursing preferred.

- Maintains confidentiality of client assessments and records.
- Utilizes time effectively, maintaining a consistent level of productivity.
- Identifies and uses appropriate resources.
- Coordinates scheduling of client’s multidisciplinary appointments and therapies.
- Consistently submits timecards and patient records completed in an appropriate timely manner.
- Consistently maintains an adequate level of supplies and equipment in working order in conjunction with suppliers and family.
- Demonstrates sound judgment and clinical knowledge in planning and decision-making.
- Consistently complies with standards for attendance, absence notification, and punctuality.
- Consistently demonstrates professionalism through performance, communication, and by adhering to the Agency dress code.
- Assumes responsibility for reading and comprehending all posted notices, communications, and policies/procedures.
- Demonstrates competencies to provide care to patients of all ages.
- Respects the rights, privacy, and property of others at all times.
- Assumes responsibility to participate in quality improvement activities as directed.
- Consistently plans and implements supportive and restorative care.
- Consistently coordinates community and social services to meet the needs of the client and caregiver/family.
- Performs other duties as assigned by OSH. Follows Federal, State, and Local laws and regulations pertaining to home care services.
