SCHEDULE:
This position offers three different schedule options after an employee's initial one-month training period.
  • Regular: Monday – Friday (8 hour days)
  • Flex: Four 9 hour days and one 4 hour day (typical schedule: Mon-Thurs 7:30-5pm, Fri 8am-12pm)
  • Compressed: Four 10 hour days (7am-5:30pm, Mondays or Fridays off)


DETAILED SUMMARY:

Provides direct case management services to individuals and families who are homeless or at risk of becoming homeless, focusing on helping them achieve housing stability. Uses Client-centered, Housing First, Harm Reduction and Motivational Interviewing models of case management that is client driven, solution focused and facilitates change and movement toward greater stability and well-being. Specific duties may include, but are not limited to home visits, intakes, the tracking of services, and assisting in program development.

Opportunity for promotion to Housing Case Manager I after successful completion of six month introductory period and demonstrated ability to perform position competencies.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
1. Provides homeless housing related case management services to families or individuals participating in homeless housing programs.
2. Conducts intakes and provides assessments.
3. Maintains confidential case notes on each individual or family client in a timely manner.
4. Develops and maintains cooperative working relationship with other agencies and service providers.
5. Serves as link between client and resources.
6. Provides reporting and tracking information for data management systems.
7. Determines client eligibility for homeless housing services.
8. Ensures that all federal, state, agency and/or funder guidelines are met.
9. Assists in program development work.

OTHER RESPONSIBILITIES:
1. Attends staff trainings and meetings as required.
2. Other related duties as assigned.
EDUCATION, EXPERIENCE, TRAINING REQUIRMENTS:


1.Bachelor’s degree in Human Services, Psychology, Sociology, or related field and six months related experience or equivalent education/experience.
2. Case management experience in social service employment setting, and a working knowledge of issues and resources in relation to housing services for low-income families preferred.
3.Experience working with persons living with mental illness
4.Experience working with persons experiencing chronic homelessness.
5. Experience working with persons experiencing substance use disorder
6.Knowledge of the Substance Use Disorder, including it's signs and symptoms.


SPECIAL REQUIREMENTS:

1. Valid Washington State Driver’s License by time of hire.
2. Good driving record. Driving abstract must be submitted upon request.

SKILLS AND ABILITIES REQUIRED:
1. Verbal and written communication skills.
2. Computer skills, which include word processing, data entry, e-mail, etc.
3. Interpersonal skills and the ability to interact positively with clients from diverse populations.
4. The ability to create an innovative approach or new procedure or system as appropriate.
5. Ability to maintain accurate and timely case management records.

WORKING CONDITIONS:
Work is generally performed in a fast–paced office environment with frequent interruptions and occasional crisis situations. Frequent home visits off site. Frequently requires travel within the agency’s service areas for home visits, community resource networking, meetings, special workshops, speaking engagements and other job-related professional activities.
