Summary
This hours for this position are 2pm-11pm.
Responsible for taking clients to medical appointments and maintaining compliance with follow up visits, including but not limited to office visits, labs, adaptive equipment. Maintain open communication with members of the interdisciplinary team regarding client's health.
Essential Functions
· Assist clients to arrive promptly to scheduled and unscheduled medical appointments with appropriate documents.
· Provide necessary direct support to help client understand doctor’s instructions.
· Communicate with doctors the reason for visit and client symptoms; express concerns and ask questions.
· Fax documents pertaining to prescription medication to the pharmacy.
· Work closely with nurses, case and house managers, and staff.
· Assist clients in self-administration of medication.
· Promote client independence and problem solving.
· Assist clients with personal care as needed.
· Advocate and protect client’s health, safety, welfare, and personal well-being; implement emergency safety plans.
· Implement behavior intervention techniques.
· Comply with all applicable policies, procedures, accreditation requirements.
· Perform other duties as assigned.

· Strong Computer skills.
· Ability to exercise good judgement and patience when working with clients.
· Ability to maintain accurate records.
· Excellent written and verbal communication skills.
· Ability to keep client and OH business information confidential.
· Ability to make last-minute changes and work on multiple tasks at a time.
Work Environment
While performing the duties of this job, the employee will work in an office environment that is quiet to moderately loud, depending on the work area. Driving clients to appointments is the primary duty of this position. Many offices are local, however there may be appointments that are in Naperville, Elgin, St Charles or Chicago, as well Employee is required to sit, use hands, stand, stoop, walk, talk, and listen while performing duties. General office equipment including personal computers, scanners, copiers, faxes, desk- and smartphones, and software programs.

The employee must occasionally lift or move up to 20 pounds. Requires normal range of hearing/vision. Must be able to physically assist mobility impaired individuals.
Required Education and Experience
· High school diploma or GED.
· Valid driver’s license is a must, auto insurance 100/300/100 (Bodily liability coverage per person, accident and property damage, and vehicle to use on company business.
· Personal telephone (cell).
· Be registered as a Direct Support Professional and be approved by our agency nurse to administer medications within three months of employment.
