Position Title: Case Manager
Employment Type: Full-Time

Position Overview:
Optima Health Care Systems is seeking a compassionate and dedicated Case Manager to provide comprehensive case management services to adults and adolescents with substance use disorders. This position will involve working in an outpatient setting to assist clients in developing and implementing coping skills for managing issues such as trauma, family or peer conflicts, mental health disorders, substance abuse, grief and loss, relational challenges, and community re-engagement. The ideal candidate will be proactive, detail-oriented, and committed to helping clients achieve long-term recovery goals.
Key Responsibilities:
- Case Management & Care Planning:
- Develop person-centered, comprehensive case management care plans within 10 days of client intake, identifying client goals, needs, and the corresponding services to be provided.
- Provide both in-person and telephonic case management services, ensuring consistent follow-up and support in alignment with the client’s established care plan.
- Conduct regular evaluations of client progress toward treatment goals and objectives, and update care plans accordingly to reflect the evolving needs of the client.
- Communicate regularly with clients’ mental health providers, probation/parole officers, community service providers, and family members to ensure continuity of care.
- Documentation & Reporting:
- Maintain accurate and thorough documentation of all case management services provided, submitting progress notes within 24 hours of service delivery.
- Participate in weekly clinical team meetings to provide updates on client progress, discuss treatment plan adjustments, and ensure coordinated service delivery.
- Complete regular updates to client care plans, reflecting changes in treatment goals or the introduction of new needs, and ensure documentation is compliant with privacy and confidentiality standards.
- Clinical Collaboration & Professional Development:
- Work collaboratively with the Clinical Supervisor to develop a Professional Development Plan within the first week of employment and adhere to all conditions of clinical supervision as outlined in the plan.
- Ensure that treatment and case management services are aligned with established best practices, including the use of ASAM criteria for assessments.
- Actively engage in ongoing training and development opportunities to improve case management skills, therapeutic knowledge, and overall effectiveness.
- Ethical & Legal Compliance:
- Uphold and demonstrate commitment to the highest standards of professional ethics, ensuring confidentiality and privacy compliance in all aspects of client care.
- Adhere to all applicable regulations related to the provision of substance use disorder treatment, ensuring the agency's practices align with current laws and ethical standards.
Minimum Qualifications:
- Education:
- Bachelor’s degree or higher in a health or human services field.
- Certifications:
- Magellan Certified Case Manager (CCM) certification (Preferred).
- Completion of training in the use of ASAM Criteria, or ability to complete this training within the first week of hire (Required).
- Experience:
- Previous experience working with substance use disordered populations is preferred.
- Demonstrated ability to develop and implement effective case management care plans.
- Skills & Competencies:
- Strong organizational skills, with the ability to manage multiple cases and meet deadlines.
- Excellent written and verbal communication skills, including the ability to accurately document services and communicate with multidisciplinary teams.
- Ability to work independently, while also collaborating effectively with clinical and community teams.
- Knowledge of substance use disorders and mental health conditions, and the ability to apply appropriate case management strategies.
- A commitment to maintaining confidentiality and adhering to ethical standards in all aspects of the job.
Preferred Qualifications:
- Experience working with both adult and adolescent populations with substance use disorders (preferred).
- Familiarity with ASAM criteria and its application in treatment planning (preferred).
- Experience in the use of electronic health record (EHR) systems for case management documentation (preferred).
Schedule:
- This is a full-time or part-time position with flexible hours, typically Monday through Friday during standard business hours. Occasional evening hours may be required depending on client needs.
