Optimae LifeServices is hiring a full-time Intake Coordinator for Burlington. The Intake Coordinator will be responsible for overseeing all incoming referrals for site services and all other programs as needed (community integration, community services, etc.). Will oversee initial meet and greet for customers the referral team would like to assess beyond initial review of identifying information. Coordination of the onboarding of new customers into the site program or other programs needed.
Duties and Responsibilities:
- Must display the aptitude and willingness to work closely with the staff and customers, excellent communication and networking skills, and use initiative, good judgement, and leadership skills.
- Use a computer regularly and have an aptitude for basic computer skills.
- Honor and respect the rights and privacy of our customers.
- Work as a leader of a team.
- Work in a stressful environment.
- Communicate with customers, co-workers, staff supervised, the public, and other providers of service.
- Maintain close communication with referral sources including IHH workers, Case Managers, local hospitals, and other outside sources of referrals.
- Coordinate with appropriate Service Coordinators.
- Coordinate any assistance that may be needed with room preparation or physical mover of customer.
- Compile customer specific initial intake paperwork.
Qualifications:
- High school diploma or GED is required.
- Bachelor's degree from an accredited college or university is preferred.
- Experience preferred in the field of human services.
