Opportunity awaits! We are hiring multiple Complex Case Managers (PreAdmission Screening Specialists) to join our team! In these positions, you will determine long term care eligibility and find placement for consumers who are incarcerated in an Oregon Department of Corrections facility, residing at the Oregon State Hospital, or who have complex care needs arere eligible for APD Medicaid long term care services. Summary Of Duties

As a Complex Case Manager, you will:

  • Determine if the consumers could be placed in their own home or the home of a family friend or relative.
  • Determine if additional caregiving hours are needed to support the individual and natural supports. If additional hours are needed, request an In-Home Exception request.
  • Evaluate the home and determine if modifications are needed to accommodate the consumer needs, such as wheelchair ramps, support bars installed in bathrooms. If modifications are needed, request K-plan funding.
  • Assist the consumer or family member with locating caregivers. Refer the consumer or their representatives to resources that will assist them in hiring a caregiver.
  • Schedule and conduct CAPS assessments for these consumers to determine if they meet service level eligibility.
  • Interview staff at the Oregon State Hospital and the Oregon Department of Corrections regarding the consumer’s activities of daily living including cognition.
  • Travel to consumers’ homes, long term care facilities or hospitals to conduct assessments and interviews throughout the state.
  • Provide on-going case management services to APD consumers with Behavioral health conditions, traumatic brain injuries or other complex care needs. This includes locating community placements and coordinating services with community partners to support the consumer.

Minimum Qualifications

A valid driver’s license and acceptable driving record are required for this position.

Six months of experience providing services to persons who are elderly or disabled AND

training and experience which shows knowledge of at least four (4) of the areas listed:

  • Medical and psychosocial problems of the elderly and disabled.
  • Normal and abnormal human development and behavior.
  • Appropriate intervention methods for the elderly and disabled.
  • Medical terminology and procedures.
  • Implications of illness or injury upon clients and families.
  • State and Federal rules about long-term care, medical services, and clients' rights.
  • Policies and procedures about agency programs.
  • Available community resources.
  • Crisis intervention, counseling, being an advocate, community relationships, and/or referral methods.

Essential Attributes

We are looking for candidates with:

  • Experience working with aging adults and people with disabilities and/or complex behavioral health needs.
  • Experience conducting a CAPS Assessment or other assessments to determine eligibility.
  • Experience locating long term care placements for aging adults and people with disabilities or vulnerable communities.
  • Experience collaborating with other units or agencies to coordinate services for aging adults, people with disabilities, or vulnerable communities.

Working Conditions

  • Hybrid work. You will work from home and have frequent travel to consumers’ homes, long term care facilities or hospitals to conduct assessments and interviews throughout the state.
  • Occasional travel throughout the state and outside of the state will be required.
  • High pressure environment.
  • Interview staff at the Oregon State Hospital and the Oregon Department of Corrections regarding the consumer’s activities of daily living including cognition.
  • Frequent collaboration with community partners to support positive outcomes for consumers.
  • Will come in contact with animals, some which of may be dangerous.
  • May be exposed to communicable diseases, including but not limited to the flu virus, Covid-19, and MRSA.
  • You will interact with a wide range of people including some who have experienced trauma and may exhibit challenges in controlling their emotions.
  • You may encounter stressful situations that require quick decision-making to ensure safety of yourself and others.

Background Checks And Requirements

  • The Oregon Department of Human Services does not offer visa sponsorship. Within three days of hire, you will be required to complete the US Department of Homeland Security’s I-9 form confirming authorization to work in the United States. ODHS will use E-Verify to confirm that you are authorized to work in the United States.
  • If you are offered employment, the offer will be contingent upon the outcome of an abuse check, criminal records check and driving records check, and the information shall be shared with the Oregon Health Authority (OHA), Office of Human Resources (OHR). Any criminal or founded abuse history will be reviewed and could result in the withdrawal of the offer or termination of employment.
  • Additionally, you must complete the Service Provider Registration process to be allowed access to the Oregon State Hospital. This includes an additional background check, agree to the Service Provider Registration terms and conditions and the Oregon State Hospital Confidentiality Agreement.
  • You may have to have a LEDS background check as required for Department of Corrections.
