The MDS/Case Management RN or LPN is responsible for the coordination of quality care to residents, from pre-admission through discharge. The MDS/Case Management RN or LPN will coordinate the written care plan and assessment and coordinate professional nursing care to residents.

Education and Experience
- Licensed RN in the State of Ohio
- Prior MDS 3.0, Case Management and LTC experience
- MDS/Case Management Nurse will interact with many people so it is imperative to have constant professionalism and a pleasant disposition
- 1 – 2 years of experience in Long Term Care preferred
- MDS and Case Management experience required
