Utilizing your expertise in motivational health coaching, and health, you will develop personalized care plans tailored to each client's needs.
Key Responsibilities:
Conduct Initial Health Assessments:
- Assess clients’ physical health, medical history, and personal goals to develop a tailored careplan.
- Gather information about the clients lifestyle, nutrition, physical activity, stress levels, and existing health conditions.
- Collaborate with healthcare providers ( Case managers, physicians, nurses, dietitians) to review medical treatments, medications, and care plans.
- Develop individualized careplans that address the client’s unique needs, focusing on lifestyle improvements, diet, exercise, mental health, and chronic condition management.
- Ensure plans align with any medical treatments or therapies prescribed by healthcare professionals.
- Monitor and Track Progress:
- Regularly check in with clients through home visits, or phone calls.
- Use tools like Alayacare app, or trackers to monitor vital signs, nutrition, and activity levels.
- Help clients identify and address obstacles to progress, adjusting the approach as needed.
- Live data entry
- Maintain open communication with case managers, physicians, nurses, etc, and other healthcare professionals involved in the client's care.
- Share updates on client progress, insights, and challenges to ensure coordinated care.
- Provide feedback to healthcare providers about the client’s adherence to health plans and any areas requiring attention.
- Education: A degree in health-related fields such as health coaching, nursing, nutrition, psychology, or exercise science is preferred. Certification in health coaching, wellness, or related areas is a plus.
- Experience: Prior experience in health coaching, nursing, personal training, is highly desired. Experience in home healthcare or working with individuals in a home care setting is beneficial.
- The role primarily involves working in client' homes, with some in office sessions.
- Preferrable someone in Fort Wayne willing to travel occasionally to Southbend area.
- Travel between clients homes may be necessary, depending on the geographic area.
Salary : $55,000 - $65,000
