Job Summary
This is a full-time position responsible for providing high-quality clinical services to patients in their homes. The ideal candidate will possess excellent communication and interpersonal skills, be able to work independently, and demonstrate a commitment to delivering exceptional patient care.
Key Responsibilities
- Assess patients' needs and develop individualized care plans in collaboration with physicians and other healthcare professionals.
- Conduct comprehensive assessments (OASIS) and accurate documentation of patient visits and medication reconciliation.
Clinical Competence
- Deliver high-quality clinical services within the scope of practice, adhering to infection control standards and payer requirements.
- Directly supervise home health aides and licensed practical vocational nurses, providing instruction and assigning tasks as needed.
Quality and Safety
- Evaluate patients' medical necessity and eligibility for home health care, accurately determining primary focus of care and developing plans within State guidelines.
- Communicate effectively with agency staff, physicians, patients, and family members to ensure quality care and service excellence.
Licensure and Certification
- Hold current RN licensure in the state of practice.
- Obtain and maintain current CPR certification.
- Possess a valid driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
