Hospice On-Call Nurse
JOB SUMMARY:
The Hospice On-Call Nurse is responsible for providing skilled nursing, intervention, and supportive care to patients in their home setting for evening, night, weekend, and holiday on-call duty, in accordance with Peaceful Hearts Hospice, LLC’s plan of care for each patient.
Organization Relationship:
The Hospice On-Call Nurse reports directly to the Director of Patient Care Services (DPCS).
Qualifications:
Education and Experience:
- Must be a currently licensed, in good standing, Registered Nurse in the state of Nebraska.
- Graduate from an accredited school of Nursing.
- Experience of three (3) years as a nurse, preferred.
- Experience of two (2) years in hospice, home health or oncology, preferred.
Responsibilities and Duties:
- Assume responsibility for evening, night, weekend, and holiday on-call duty as scheduled, and preform care consistent with Peaceful Hearts Hospice, LLC policies and procedures.
- Initiate communication with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Assume primary responsibility for total census during assigned shift, including assessing, planning, implementing and evaluation phases of the nursing process.
- Maintain regular communication with Registered Nurse Case Manager (RNCM), attending physicians, and DPCS concerning patient/family care needs and response to interventions.
- Respond to medical or emotional crisis and/or death of a patient and make home visit to provide direct care as appropriate.
- Meet regularly with hospice nursing staff to review problems or unique issues from on-call hours, share professional support, and exchange feedback aimed towards enhancing professional growth.
- Attend Interdisciplinary Team (IDT) meetings and facilitate discussion of issues from on-call hours for full staff discussion, consultation, and evaluation.
- Perform specific assignments as directed by DPCS.
- Respond immediately to on-call phone calls, unless with a patient or family member and unable to step away. Ensure appropriate back up has been established.
- Communicate significant findings, problems, changes in condition or environment to the RNCM, DPCS and attending physician.
- Answer all incoming calls in a manner appropriate for the nature of the call. Respond to all calls with a follow-up call and in person visit for assessment, education, personal cares and symptom management when necessary.
- Complete referrals, admissions, and death visits.
- Share in on-call rotation, providing 24-hour/7-day-a-week coverage to patients/families when necessary.
- Cover on call support when needed in the event of another staff members absence.
- Provide appropriate pain/symptom management as prescribed by ordering physician.
- Maintain up-to-date patient records using computerized medical record system so that problems, plans, actions, and goals are accurately and clearly stated, and changes are reflected as they occur.
- Manage and document phone calls from physicians, clinicians, patients, and referral sources, and communicate patient updates/new orders to team.
- Perform specific nursing procedures (treatments, symptom management, wound care, tube feeding/care, bowel regimen, etc.) as ordered by prescribing physician.
- Provide care in all settings, including but not limited to skilled nursing facility, assisted living, independent living, or any home setting.
- Provide emotional support at time of death to family and caregivers.
- Pronounce Time of Death in accordance with county and agency policy, complete necessary documentation and notify MD and family.
- Ensure patient needs are met with adequate supplies and that medications are available.
- Educate patients and families on end-of-life disease processes and symptoms to expect/report.
- Conduct informational meetings with patients and family members after hours and on weekends.
- Compete consents with patient and/or Power of Attorney (POA) when needed.

- Maintain and encourage open communication with employees without allowing personal biases and/or belief to interfere with the relationship.
- Maintain professional licensure at all times. Report any disruption in professional licensure immediately to department head.
- Maintain CEU and on-going education.
- Comply with infection control policies and protocols.
- Promote hospice referrals within the community.
- Participate in agency and community programs as requested to promote professional growth and educate the community on the meaning and benefits of hospice care.
- Knowledgeable of hospice diagnoses and qualifications of hospice care.
- Understand the philosophy of the hospice concept and needs of the terminally ill.
- Educate patient and families on end-of-life disease processes and symptoms to expect/report.
- Participate in group memorial services for staff, the bereaved, and the community.
- Able to work in a highly stressful atmosphere.
- Maintain professional boundaries at all times.
- Available at all times during business hours; unless time off is pre-approved by department head and a replacement is identified.

Working Environment:
The Hospice On-Call Nurse must be actively involved in all areas of patient care. The Hospice On-Call Nurse will primarily be in the field; however, will spend time in the office for meetings, charting, phone calls, etc.
The Hospice On-Call Nurse position requires the ability to utilize a computer, calculator, cell phone, multi-line telephone, and other related office and nursing equipment.
The Hospice On-Call Nurse has access to all patient medical records.
The Hospice On-Call Nurse must have good time management skills.
Transportation:
The Hospice On-Call Nurse must have a valid driver's license, automobile liability insurance and reliable transportation.
Physician/ADA Requirements:
The Hospice On-Call Nurse will exert physical efforts on most days which may include kneeling, squatting, reaching, twisting, climbing, walking, and exposure to temperature and humidity changes, and may include lifting and/or transferring a patient when providing patient care.
The Hospice On-Call Nurse must possess sight/hearing or use appropriate adaptive devices.
The Hospice On-Call Nurse must provide evidence of an annual TB test and other state-required tests and examinations.
