Job Overview
Performs moderately complex Enhanced Community Coordination (ECC) and Habilitation Coordination case management/monitoring throughout Pecan Valley Centers’(PVC) six-county region in the Preadmission Screening & Resident Review program (PASRR). Work involves developing and maintaining long-term contact with clients, client families, and service providers for medical, social, educational, and related service needs.
ESSENTIAL JOB FUNCTIONS
Under the direct supervision of the PASRR Program Manager and Enhanced Community Coordinator (ECC) Supervisor, the Enhanced Habilitation Specialist (EHS) provides ongoing Habilitation coordination/Enhanced Community Coordination services for clients in the community at large or residing in nursing homes within the PVC catchment area. The EHS must complete all ECC activities including post move monitoring and Service Planning Team (SPT) attendance/documentation. EHS will also conduct monitoring, complete relevant documentation, and convene SPT attendance for PASRR individuals receiving Habilitation coordination services. The EHS makes referrals and coordinate services for nursing home residents and the Intellectual or Developmental Disabilities (IDD) Target population seeking Home Community-based Services (HCS) or Nursing Facility (NF) diversion in the community. EHS maintains face to face contact with all clients, families and providers as needed and documents contact in chart.
Job Duties
Provides ongoing Habilitation Coordination (HC) and Enhanced Community Coordination (ECC). This position will complete the Preadmission Screening & Resident Review program (PASRR) Habilitation Assessments/Evaluations within proper time frames. This position completes the Hab package, Community Living Options (CLO), the PASRR Comprehensive Service Plan (PCSP), Post move monitoring visits, Interdisciplinary Care Team (IDT) meetings, and Service Planning Team (SPT) meetings with all participants in team meetings at the nursing facility or in the community. Maintains face to face contact with all assigned individuals, families (as appropriate), and providers as needed. Duties include but are not limited to the following:
- Maintain understanding of All PASRR and ECC services and UM guidelines for the assigned levels of care.
- Accurately completes PASRR Evaluation (PE) and Habilitation Assessments (HA) as needed and to identify an individual’s needs/strengths as appropriate.
- Must be able to utilize an electronic health record system to document collaboratively and concurrently with consumers across all processes, including assessment, service planning, and ongoing consumer-provider interactions.
- Meet productivity standards set by the agency.
- Communicate with referring entities, internal and external providers.
- Maintain a flexible work schedule while demonstrating effective time management to meet needs of individuals and facilities served.
- Participate in staff meetings as scheduled.
- Participate in data collection and peer review processes as requested.
- Maintains a PASRR caseload of 10 to 13 individuals and an ECC caseload of up to 10 to 13 individuals (starting from Post move monitoring and concluding after 365 days of ECC monitoring via the Service Planning Team).

Qualifications

Bachelor’s degree in a human services field from an accredited college or university.

Minimum of one to two years of experience with the IDD population in a community or in a residential setting is required.

Bilingual ability (Spanish/English) is preferred but not required.

Must have a valid Texas driver’s license, a good driving record for the past three years, and personal automobile liability insurance as required by the state of Texas.
