About the Role :
We are seeking a highly skilled MDS Coordinator- RN to join our team at Pioneers Skilled Nursing Center. As the MDS Coordinator, you will be responsible for ensuring the accuracy and completeness of patient assessments, care plans, and documentation. You will work closely with our interdisciplinary team to develop and implement care plans that meet the individual needs of our patients. Minimum Qualifications :
- Active RN license in the state of California
- Minimum of 2 years of experience in MDS coordination
- Knowledge of state and federal regulations related to long-term care
