Platinum Community Care has been providing behavioral health care in the form of social workers and case managers to the NYS Hotels serving the Migrants/Asylees.
Overview:
We are seeking a dedicated Case Worker to join our team assisting asylum seekers under New York City Mayor Eric Adams' program with the Housing Preservation and Development Department, based at hotels in the Albany area. Case Workers are a member of an interdisciplinary team to provide case management to migrant, asylee or homeless based individuals with medical, behavioral or social issues or disorders.
Ideal candidates will be able to effectively advocate for and support the client, engage with community agencies/health care providers and others on his/her behalf to ensure access to services needed to increase wellness, self-management and reduce emergency room visits and/ or hospitalizations.
Responsibilities:
- Responsible for performing a variety of casework duties, including meeting with guests, and preparing evaluations, including obtaining, analyzing, and evaluating data and developing services plans.
- Responsible for making the residents aware of the facility's rules and regulations and providing information about the services offered.
- Performs record keeping functions as required, case notes, census statistical reports while making sure all records are maintained at a confidential level.
- Refers guests to agencies and programs as necessary and establishes a rapport and professional relationship with all stakeholders.
- Assists guests in establishing relationships with outside services. Assists in arranging transportation to necessary appointments.
- Prepares a facility workshop and meeting with clients relative to goals of Social Services and needs of residents and works closely with other personnel assigned to residents.
- Handles emergencies as they arise and arranges for medical services or guidance to the medical team.
- Attend regular staff meetings and document progress to the supervisor.
Requirements:
- HS Diploma or higher.
- Preferred: Bachelor's Degree in one of the following fields: Social Work, Psychology, Education,
- Rehabilitation, Occupational Therapy, Counseling, Community Mental Health, Sociology, Physical or Recreational therapy. Degrees in other related areas may be considered.
Experience:
- One (1) year of related human services experience required in providing direct services to clients suffering from or experiencing medical, behavioral or social issues and knowledge of resources, in order to link them to a broad range of services essential to successfully living in the community.
- You must have excellent interpersonal and time management skills.
Preferred:
- Bilingual in Spanish, Haitian Creole, Georgian, Russian, Woluf, Arabic, Hindu a HUGE Plus
