- Develop and implement effective outreach strategies to engage with diverse communities and promote our health and wellbeing services.
- Build and maintain strong relationships with community partners, including health centers, providers, councils on aging, and social service organizations.
- Conduct telephonic and face-to-face interactions with member/caregiver(s) and provider partners to ensure smooth onboarding and ongoing support for members.
To succeed in this role, you will require:
- A minimum of 3 years of professional experience in a managed care company, medical-related field, or community social service agency.
- Excellent written, oral, and interpersonal communication skills, and the ability to convey complex information in a clear, easy-to-understand manner.
- Ability to work compassionately and efficiently with diverse populations, and adapt to changing priorities and deadlines.
