Job Summary: 


The  Prevention Specialist assigned as the Youth Program and HIV Navigator provides HIV/STI testing, PrEP enrollment, HIV prevention services for youth and assists in the coordination of the Youth Program and the LGBT Safe Space. 


 

Requirements

  • Conducts HIV Counseling (CTL) sessions in a non-judgmental manner and with fidelity to the core elements and core components of the HIV Prevention Counseling standards.
  • Provide effective condom education. Distribute condoms to target HIV-infected persons and persons at highest risk of acquiring HIV.
  • Conduct a financial assessment to identify available financial resources for the client and identify what additional resources the client may need. 
  • Assist Clients with completing any financial forms for PrEP Medication access. 
  • Provides education on pre-exposure prophylaxis (PrEP) and ensures persons eligible for PrEP if interested are linked to PrEP services to include Agency intake paperwork.
  • Abide by all state, health district and agency policies regarding confidentiality of patient information
  • Works with supervisor on a regular basis to adjust work schedule to accommodate the current HIV/STI screening needs of the department. 

Youth Program 


  • Develop and implement a weekly drop-in group with an average attendance of the 10 youth per session. 
  • Provide outreach to youth that includes one-on-one rick reduction conversations and condom distribution in the community setting. 
  • Assists in the planning and implementation of at least four HIV Prevention awareness events, in-person or virtual, resulting in an increase of youth accessing a prevention service. 
  • Produce at least two youth-focused media clips per month, including videos, podcasts, and photos that will expand the visibility of the PrEP program, department services, youth program, as well as other agency services. 
  • Supports the facilitation of the other elements of the youth program when needed. 
  • Be an additional prevention resource for engaged patients and clients. 
  • Recruits and links on average at least two youth per month into PIHC PrEP services. 
  • Make appropriate referrals for mental health care, primary healthcare, PrEP access, substance abuse treatment, or other social service referrals, and follow and track those referrals to ensure access. 
  • Works with supervisor on a regular basis to adjust work schedule to accommodate the current youth programming needs of the department. 


Requirements

 Knowledge, Skills, and Abilities 

  • Must possess excellent interpersonal skills and the ability to communicate effectively in a multi-cultural setting, including diverse clients, volunteers, agency staff, and Board of Directors 
  • A successful candidate will  be a self starter with the ability to multi-task. 
  • Possess excellent communication skills (writing, spelling, listening and speaking). 
  • Cooperates as a team member. 
  • Possesses problem-solving and decision-making abilities. 
  • Ability to work without close supervision. 
  • Utilizes good management techniques. 
  • Follows through to completion of assigned tasks. 
  • Proficiency in word processing, spreadsheet database and presentation software. 
  • Abides by all policies and procedures concerning personnel, including conflict of interest policy. 
  • Knowledge of HIV/AIDS, knowledge of target population, and past direct experience with People living with HIV/AIDS a plus. 
  • Due to nature of the work, the applicant must pass a police background check and drug screening. 
  • Remain current on HIV/AIDS issues and specifically on PrEP and how it is implemented across  the country. 
  • This position will require working some events, weekends, and holidays based on a weekly schedule provided by the program manager. 


 Minimum Qualifications 

  • Associate's degree from a accredited institution; Bachelor's degree preferred, in public health, counseling, psychology, sociology, or related field. 
  • Must have access to reliable transportation for work related purposes. 
  • Previous experience working in interventions and/or programming preferred. 

Experience 

  • Two years of experience working in the field of HIV, PrEP or public health or any equivalent combination of proven training and experience which provides the required knowledge, skills and abilities to preform this job
