PRC Baker Places is a non-profit residential treatment agency that has provided the San Francisco community with alternatives to institutional care since 1964. Our direct care professionals provide counseling and case management services to previously homeless adults living with co-occurring diagnoses of chronic mental illness and substance use disorder. Position Overview
The Key Program Assistant works as a member of a team to help build better connections and treatment follow-up for people who received services from the HIV / Hep C testing grant who are out in the community or other residential treatment settings. Under the direction of the Project Director, the Key Program Assistant is responsible for providing administrative and programmatic support for the grant.
The current Grant is in place until April of 2027, and is a Exempt, Full-Time, Fully Benefited position with the agency.

Primary Duties and Responsibilities
- Actively participates in the development and implementation of the project
- Provides orientation and conducts intakes for new participants
- Assists in scheduling and completing follow up appointments with participants
- Assists in locating participants
- Attends all meetings, trainings, collaborative efforts, and other project and agency-related functions as required
- Maintains confidential participants records, files, and other sensitive information both electronically and in paper form
- Collects and reports at intake / enrollment, completion, and at follow-up to support continued funding for the service
- Other duties as assigned
