Key Responsibilities:
- Provide strategic direction for clinical care coordination, discharge planning, and case management services.
- Collaborate with interdisciplinary teams to improve care transitions, reduce readmissions, and enhance throughput.
- Manage departmental resources, budgeting, and quality control while ensuring regulatory compliance.
- Serve as a resource to senior leadership, staff, and interdisciplinary teams, promoting innovation and evidence-based practices.
- Facilitate performance improvement processes and align departmental operations with organizational goals.
Qualifications:
- Experience:
- 5 years in case management, discharge planning, or cost/quality management.
- 3 years in a hospital clinical role.
- Supervisory/management experience required.
- BSN required; Master's degree preferred.
- Active Ohio RN License (Required).
- BLS (AHA) Certification (Required).
