Position Summary:
The COTA will work under the supervision of the occupational therapist to deliver patient-centered therapy services in a home health setting. This role involves implementing treatment plans, documenting patient progress, and collaborating with our multi-disciplinary team to ensure the best outcomes for patients.
Key Responsibilities:
- Implement occupational therapy treatment plans as directed by the Occupational Therapist (OT).
- Provide hands-on therapy to patients in their homes to improve activities of daily living (ADLs) and functional independence.
- Document patient visits, progress, and outcomes accurately and promptly in accordance with regulatory standards.
- Educate patients and caregivers on therapeutic techniques to enhance recovery and maintain functional abilities.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
Qualifications:
- Current Wisconsin licensure as a Certified Occupational Therapy Assistant (COTA).
- Minimum of 1 year of experience as a COTA, preferably in a home health setting (new graduates with strong clinical experience will be considered).
- Valid driver’s license and reliable transportation.
- Strong communication and organizational skills.
- Ability to work independently while maintaining a collaborative team approach.
