Job Summary
The role of the RN is to ensure that the health and safety of all residents are met through assessments of the residents, development and implementation of appropriate plans of care, supervision of residents’ services, supervision of assisted living staff and on-going training of assisted living staff. This position reports to the Executive Director and directs the caregiver and nursing staff.
Essential Functions
Responsible for the oversight of the assisted living/nursing department under the comprehensive license including any arranged assisted living relationships to include the following functions:
- Completes initial assessment of prospective residents and develops a proposed service plan prior to the date on which a resident signs a residency agreement or moves in, if possible. If a face-to-face assessment is not possible prior to the date the resident signs a residency agreement or moves in, the RN will talk via telephone with the prospective resident, the resident’s responsible person and/or current health care providers, if possible, to provide information about the assisted living services available to residents and to gather information about the resident’s needs prior to the date on which the resident signs a residency agreement or moves in.
- Completes assessments on or before day of move-in and before any delegated nursing services are provided to the resident
- Implements service plans with all new residents. Reviews and revises service plan as needed due to changes in residents’ needs and a minimum of once every 12 months
- Assures residents have up-to-date care plans. Reassesses the resident and modifies the service plan if necessary whenever there is a change in the resident’s condition or if the resident or the resident’s responsible person requests a change in services.
- Monitors and maintains resident’s clinical chart and clinical progress notes.
- Communicates to physicians, family or responsible party, and other personnel (e.g., county caseworkers, health plan care coordinators) changes in resident’s needs or conditions.
- Facilitates coordination of resident’s assisted living services with other service providers.
- For any resident receiving assistance with medications, the RN is responsible for ordering (or delegating to an LPN) medications and assuring medication orders are renewed annually. Is responsible for ensuring any necessary follow-up with physicians regarding medications and/or medical conditions is completed in a timely manner.
- Assures all physicians orders are signed and implemented within 24 hours of receipt.
- Assures that proper systems, policies and procedures are developed and implemented correctly by licensed and unlicensed staff regarding the storage, set up, administration and disposal of medications, including systems to control Schedule II - IV medications.
- Sets up medications, or delegates task to an LPN, on a specified routine for medication administration by unlicensed staff and makes changes as necessary to comply with new orders.
- Assures that medication administration procedures are followed properly, investigates any errors and issues with staff, and implements corrective action plans as needed.
- Communicates in resident’s file and 24 hour report any information that staff need to be aware of.
- Reviews the staff communication log (24 hour report) each work day.
- Documents disposal of medication per policy.
- Performs a reassessment a resident no later than 14 days after admission, at least every 90 days thereafter based on the resident’s needs, and as needed to evaluate each resident’s assisted living services. Both direct and indirect supervisory visits will be completed.
- Directly supervises LPNs’ and unlicensed staff’s performance of delegated nursing services. Conducts evaluation of staff performance of delegated nursing services in coordination with the executive director through appropriate documentation and consistent with agency personnel policies.
- Must be able to relate professionally with residents, staff and family members, display a positive work attitude and be able to work as a team with other staff members.
- Is responsible for implementing the agency’s infection control plan and TB prevention plan, including performing and documenting TB screening of employees.
- Provides all new assisted living employees with required training on orientation to assisted living requirements, “core competencies,” and competency evaluations for delegated nursing tasks (such as medication assistance) that may be necessary prior to any delegation of services. Will also provide continuing education for all staff and documents such training to assure compliance with state, federal and local regulations. Each unlicensed staff person must have at least eight hours of in-service training for each 12 months of employment. All assisted living staff must receive an annual infection control in-service as required in the assisted living rule.
- Assures that each assisted living staff person is oriented to the individual needs of each resident the person will serve.
- Assures that staff clearly understands and follows all resident care procedures or care plans.
- Monitors staff for accuracy of monthly resident blood pressures, weights and pulse as requested.
- Shares resident and family concerns with pertinent staff as appropriate.
- Supervises medication practices and medical equipment being used in the building.
- Assists the manager/administrator/owner in the development and implementation of policies and procedures related to assisted living services.
- Works with executive director to assure emergency procedures are in place and staff is trained on these procedures.
- Is flexible and willing to adapt to changes in our program as they occur to respond to the market
- Promotes a positive, team approach and a healthy work environment.
- Is available for on-call medical questions and/or emergencies via phone as assigned. On-call duty may require the RN to come to the building to handle a medical emergency. The 24/7 on-call duties can be shared between other RN’s in the facility per their job description. The Clinical Nurse Supervisor is responsible to develop the on-call calendar and post for staff.
- Assures compliance with regulations and is available for state licensure surveys.
- Identifies as early as possible when resident’s needs are likely to exceed the services that our assisted living agency can safely provide; communicates these concerns to the resident and responsible person both verbally and in writing, and provides assistance to the resident in obtaining other needed services.
- Responsible for investigating and reporting incidents of suspected resident neglect and/or abuse and reporting these findings to appropriate county officials, in coordination with the Executive Director and corporate nurse.
- Strives to maintain a safe working environment through the prevention of accidents, the preservation of equipment, and the achievement of safe working practices.
- Responsible for nursing department staffing to include interviewing, job offer, onboarding, training, in conjunction with Executive Director and other team members.
- RN Directors must have an active role in the nursing staff schedule, staffing levels and have a clear understanding of managing overtime.
- Approves absence requests, sick leave, etc., for nursing employees
- Responsible for timely job performance reviews on or before their due dates.
- Implements a daily stand-up huddle with department staff as a way of communication.
- Perform purposeful daily rounding with the intention of looking at unit/department organization, unit/department cleanliness, equipment function, potential safety issues, supply storage, repair needs and overall unit/department appearance.
- Makes staff engagement a daily priority
- Is flexible in work hours to ensure staff engagement and oversight is completed on all shifts
- Manages nursing department fiscal activity
Certifications, Licenses, Registrations and Other Requirements
- Must be registered and licensed with the State of Minnesota as a RN, and must maintain current license
- Must receive a “not disqualified” criminal background result from Department of Human Services.
- Have two years of work experience related to Alzheimer’s disease or other dementias, or in health care, gerontology, or another related field
- Previous assisted living or public health nursing experience is preferred and beneficial.
- Must be screened for TB.
