Mobile Mobile Nurse Practitioner - Vancouver, WA
Shift: Days
Medical Specialty: Geriatrics & Primary Care
Job Summary & Responsibilities:
Adult family homes, memory care and assisted living communities(within 25 miles of your home)
Manage complex patients, focused on reducing hospitalizations and improved health outcomes.
Monday- Friday schedule with an ability to schedule patient appointments into condensed workdays as desired.

Education, Licensure, and Experience:
Current unrestricted NP/PA-C license to practice in the state.
Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification
Intermediate computer skills and proficiency required
Home Office, High Speed Internet and Reliable Transportation Required.
2 years' experience as Nurse practitioners
Experience in managing chronic diseases (CHF, hypertension, diabetes etc.)
