POSITION DESCRIPTION:
The Case Manager is supervised by the Case Manager Supervisor and is responsible for ongoing needs assessment, individual treatment plans, implementation, support, information, referral and advocacy for individuals who are in the Providence House DHS program. Keeping clear and accurate files and records is essential to this position. The position will require occasional evening and weekend work hours.
RESPONSIBILITIES:
Client/Service Delivery
- Responsible for assessing the needs of the resident, preparing treatment plans ensuring needed services is provided to the resident.
- Meet weekly with residents to discuss progress on treatment goals, development of life skills and provide supportive environment for the discussion of life issues, which includes housing entitlements, employment, counseling, substance abuse programs, and education and parenting skills.
- Conduct intake interviews, provide general information, gives tour of house, explain house rules, expectation of behavior, and emergency procedures.
- Arrange meetings with housing specialist and employment specialist that will assist residents in securing housing, employment and other services.
- Prepare resident to negotiate with agencies, i.e. housing, employment, welfare, education.
- Complete bi-weekly independent living plan.
- Maintain ongoing contact with DHS concerning each resident’s progress, and participates in regular DHS meetings as scheduled.
- Enter and maintain current client information in the DHS CARES case management system.
- Maintain a current list of community resources and ensure that residents receive needed services.
- Advocate and escort clients to outside service agencies when necessary.
- Maintain accurate case records and prepare required forms and reports for outside agencies.
- Participate in individual supervisory sessions and case reviews with the Case Manager Supervisor and Program Leadership
- Attend all mandatory trainings, workshops and meetings as required.
- Meet with DHS Program staff, Core Community and other pertinent staff on a regular basis to discuss matters of mutual concern.
- Performs other duties as assigned by Case Manager Supervisor.
Sanctuary Model
- Effectively use and practice Sanctuary Models tools; safety plans, community meetings, psycho-educational groups, team meetings, red flags reviews, and self-care in the program to create a non-violent and trauma sensitive culture.
- Promote and model the core values of Providence House Mission (Compassion, Non-Violence, Hospitality, and Community) and the Seven Sanctuary Model Commitments to create a non-violent and trauma sensitive culture.
- Integrate the Sanctuary Model SELF (Safety, Emotional Management, Loss and Future) framework into daily practice to ensure quality care and an effective work environment.
Team/Staff Relations
- Work as a Providence House team member with all staff in order to implement the mission and core values of the agency.
- Attend regular internal and external agency meetings.
- Work as part of a team to plan and develop resident programs.
Administrative
- Keep accurate, up-to-date documentation on all clients. Including but not limited to, treatment plans, case notes, etc.
- Complete all paperwork in a timely and accurate manner that ensures payment to the agency from various sources.
- Assists with any and all data collection systems.
Travel
- Requires travel with clients to outside agencies when necessary.
- Position requires travel to Providence House residences and outside meetings, workshops and conference with outside agencies.
QUALIFICATIONS:
- Bachelors’ Degree, Social Work or related field.
- Bilingual in Spanish required.
- Two (2) years of case work experience required.
- Experience working with the homeless population.
- Excellent verbal and written communication skills and the ability to work independently.
- Excellent interpersonal and organizational skills.
- Competent technology & management skills, including proficiency with using Microsoft Office; and client database management.
- Required travel to our Brooklyn location as well as occasional travel throughout NYC.
WORK SCHEDULE:
The position is a full-time, salaried, non-exempt position. This position has a staggered schedule covering one weekend day (for example Tuesday to Saturday) generally 9am-5pm and/or has one day per week where the employee arrives late and stays late (for example 12pm to 8pm) to be accessible to clients with weekday obligations. This schedule can be agreed upon with the direct supervisor. The office location for this position is Brooklyn, New York.
SALARY AND BENEFITS:
This position is full-time salaried, non-exempt and earns between $50,000.00- $55,000 annually. Benefits include 14 paid holidays, 3 weeks of vacation (with additional accruals based on agency tenure), 3 personal days, 12 sick days, early closure at 12pm on Fridays proceeding Monday holidays, and a day off annually in recognition of your Birthday.
