Job Title : Journey Clinic Registered Nurse
Summary
This is a comprehensive nursing care position for patients in an ambulatory care environment. The successful candidate will provide individualized care based on physical, emotional, and spiritual needs throughout the patient's lifespan.
Responsibilities
As a Journey Clinic Registered Nurse, you will :
- Assess, identify, plan, implement, and evaluate all aspects of care and teaching needs via direct and telephonic / telehealth or electronic encounters.
- Facilitate care coordination using the nursing process to ensure seamless patient care.
