About the Role
We are seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team at Providence Benedictine Nursing Center in West Salem, Oregon. As a Home Health RN, you will provide high-quality patient care in the comfort of patients' homes, working collaboratively with other healthcare professionals to ensure seamless transitions and optimal outcomes.
Job Summary
This is a full-time position, Monday through Friday with rotational weekends. The ideal candidate will have at least one year of medical/surgical experience in an acute care setting and a strong passion for delivering person-centered care.
Responsibilities
- Conduct thorough assessments of patients' physical and emotional needs, developing individualized care plans that address these needs and promote independence.
- Administer medications, treatments, and therapies as prescribed by physicians, ensuring accuracy and safety at all times.
- Collaborate with patients' families and caregivers to educate them on self-care techniques, medication management, and disease prevention.
- Provide wound care, ostomy care, and other specialized services as needed.
- Document patient interactions, progress, and any concerns in a timely and accurate manner.
Requirements
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification within 30 days of hire.
- At least one year of medical/surgical experience in an acute care setting.
Preferred Qualifications
- Recent Home Health experience.
- Hospice or Palliative Care experience for Palliative Care RN work assignment.
