Job Title: Home Health RN
Job Summary:
We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at Providence Benedictine Nursing Center. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with patients, families, and healthcare teams to develop personalized care plans.
Responsibilities:
- Conduct comprehensive assessments of patients' physical and emotional needs
- Develop and implement individualized care plans that address patients' goals and priorities
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians
- Collaborate with healthcare teams, including physicians, therapists, and social workers, to ensure seamless coordination of care
- Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress
- Maintain accurate and detailed records of patient interactions, assessments, and care provided
Requirements:
- Current Oregon Registered Nurse License
- National Provider BLS - American Heart Association certification within 30 days of hire
- 1 year of Medical/surgical experience in an acute care setting; 2 years of experience in acute care or community mental health setting for Mental Health RN work assignment
Preferred Qualifications:
- Recent Home Health experience
- Hospice or Palliative Care experience for Palliative Care RN work assignment
- Point of Care experience
