Job Summary:
This is a Home Health RN position at Providence Benedictine Nursing Center. We are looking for a skilled Registered Nurse to provide high-quality patient care in the comfort of their own homes. The ideal candidate will have experience in home health, medical/surgical experience in an acute care setting, and be able to work independently with minimal supervision.
Job Description:
The Registered Nurse Home Health is accountable for direct and indirect quality care for patients of all ages, according to the individualized needs of the patient, following the nursing process and in accordance with the Nurse Practice Act. Professional development is an integral part of this nursing practice. Position coordinates patient care activities with other members of the health care team in a collegial relationship.
Responsibilities:
- Provide high-quality patient care in the comfort of their own homes
- Conduct thorough assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless care transitions
- Manage patient medications, treatments, and equipment as prescribed by physicians
- Maintain accurate and timely documentation of patient information
Requirements:
To qualify for this position, you must have:
- A current Oregon Registered Nurse License
- A minimum of 1 year of medical/surgical experience in an acute care setting
- Experience in home health or community mental health settings is preferred
- Ability to work independently with minimal supervision
