What are the responsibilities and job description for the RN Case Manager Home Health position at Quality Life Services?
Quality Life Services is a growing home health agency based out of Fairfield, Ohio. We are currently looking for a passionate Registered Nurse Case Manager in Hamilton County extending from Norwood to Mason. As Case Manager you would be responsible for to oversee your patients individualized plan of care from admission to discharge with assistance from PT/OT/ST and MSW as needed.
Responsibilities and Duties
· Assess the patient’s physical and functional needs
· Perform a home safety assessment
· Conduct a complete medication review
· Work with patients and families to best meet the patient’s needs, as well as the needs of the people that support them at home
· Establish a personalized plan of care
· Monitor and supervise the care provided by other nursing staff
* Wound care
* Medication education
* DX education
* IV therapy
Qualifications and Skills
· Registered Nurse in the state of Ohio
· Home Care experience is preferred, but not required
· Minimum 1 year experience as a Registered Nurse
· Must be available for infrequent weekend rotation with other RNs
· Reliable transportation with proof of insurance'
