Job Description
Quality of Life Counseling Center is a mental health agency located on both the Peninsula and South-Side areas of Hampton Roads Virginia. Quality of Life Counseling Center offers an array of career opportunities for our family oriented staff to include the following services: Intensive In-Home, Mental Health Skill-Building, Mentoring, Life-Skills Coaching, Anger Management, Substance Abuse, Vocational Training, Group Mentoring/Counseling, and more. It is our belief to diversify our services as an agency to ensure we can deliver the wrap-around services our community so desperately needs.
The Qualified Mental Health Professional is responsible for conducting direct services to the clients based on their ISP and under the supervision of the Clinical Supervisor. The QMHP will provide services according to the service description, individual service plan and the mission of the agency.
Position Responsibilities
- Assist in the development and ongoing monitoring and implementation of client centered individual service plans.
- Work with clients to foster the development of new improved coping, problem solving, and communication skills.
- Maintain up-to-date client records, and prepare required/necessary reports in a timely and accurate manner.
- Maintain coordination of care with authorized individuals and key stakeholders in planning and service delivery to include: social workers, psychiatrists, courts, and other human service professionals.
- Educate client and if applicable caregivers/family members regarding existing community services, and act as an advocate in connecting client/families to these services.
- Maintains regular and ongoing contact with referring agencies, as well as other agencies involved in youth and family’s case.
- Provides linkage to community-based services and supports; provides advocacy and support for family with these services.
- Responsible for the proper and timely documentation of all information required by DMAS, DBHDS, MCO's, and additional contracting agencies.
- Attends and participates in team meetings once per quarter.
- Attends and participates in orientation, in-service training, and conferences as scheduled.
- Attends weekly supervision and group supervision when scheduled.
- Maintains yearly certifications (CPR/First Aid, Behavioral Management, Human Rights), etc.
- Available for 24-hour crisis intervention and family support.
- Performs other duties as assigned.
Job Types: Part-time, Contract
Pay: $22.00 - $26.00 per hour
