What are the responsibilities and job description for the Community Health Worker (CHW) position at Randstad Healthcare?
We are seeking a dedicated Community Health Workers to join our team. The ideal candidate will play a crucial role in promoting health education and providing support to individuals within the community.
Majority of this role will be out in the field with the remaining time in the home office completing documentation and conducting telephonic outreach.
Responsibilities:
- Community Health Workers (CHW's) work to promote healthcare equity and assist in addressing Social Determinants of Health for our Maryland Medicaid members
- CHWs help connect our members with community resources (ex. Finding health care providers, arranging transportation to appointments, connecting with a food pantry), provide member education related to their health care and available benefits , and identify ways to improve their access to care
- Role will be outreaching members to see how we can help meet their identified needs, and will be working in conjunction with our care management department
Requirements:
- Bilingual English/ Spanish
- Maryland State Community Health Worker (CHW) Certification
- Previous CHW experience
- Experience working with Medicaid populations in urban or rural environments or experience working with our maternity population including pregnant women who might be using opioids preferred
- Proficiency with computers/technology
Schedule: Monday - Friday 8-5
Job Types: Full-time, Contract
Pay: $32.00 - $35.00 per hour
