What are the responsibilities and job description for the Birth to Three Service Coordinator position at RCS Empowers?
CLASSIFICATION: Non-Exempt
QUALIFICATIONS: Bachelor’s Degree or equivalent with experience related to service coordination and family support. Experience with home visitation, early childhood education or early childhood development is a plus. Must have good driving record, valid Wisconsin driver’s license, and required level of personal auto insurance. Use of personal vehicle is required.
REPORTING SUPERVISOR: Manager of Birth to Three
GENERAL DUTIES: Coordinate Birth to Three services to assist families in supporting their infant’s or toddler’s development in the context of naturally occurring relationships, routines, and environments through a process of assessment, planning and intervention. Guide the early intervention team through consultation, observations, reviews of progress records and direct guidance and support to ensure individual and program outcomes are being met.

SPECIFIC DUTIES:
- Secure intake information including social and medical history. Describe the purpose and scope of early intervention to the family.
- Maintain central files and prepare reports in accordance with program policy and agency regulations ensuring that specific timeframes are met.
- Coordinate and interpret standardized developmental evaluations and informal assessments in the areas of motor, language, cognition, activities of daily living, and social skills. Assess family needs and problems related to the developmental progress of enrolled children.
- Facilitate and develop Individualized Family Service Plan (IFSP), along with the child’s team, utilizing evaluation results and the Routines Based Interview.
- Ensure that the Individualized Family Service Plan (IFSP), containing measureable objectives, strategies and techniques, is carried out by team members and that the supports and services are achieving the identified outcomes.
- Lead early intervention team meetings and individual family service plan conferences.
- Provide support to families by promoting parent education programs and support groups and by implementing family activities to encourage socialization and networking.
- Gather medical information and share developmental progress with identified team members including physician’s offices, day care centers, and other agencies.
- Coordinate early intervention services with biological parents, foster parents, and social service agencies for children in the foster care system.
- Facilitate transition to school services at age three, if needed.
- Assist parents in identifying and utilizing community, state, and national resources and inform parents of the availability of advocacy services.
When appropriate, accompany families to appointments to assist in interpreting and receiving information and to assist in advocating for child’s and family needs.
- Complete 100% time reporting record on a daily basis to maximize reimbursements for services.
- Assume additional duties as directed by the Manager of Birth to Three.
