JOB DESCRIPTION: Case Manager
CLASSIFICATION: Non-Exempt
QUALIFICATIONS: Baccalaureate Degree or equivalent of experience related to the field of rehabilitation. Must be authorized for coverage under agency insurance if operating agency vehicle, have good driving record, valid Wisconsin driver's license, and/or required level of personal auto insurance.
REPORTING SUPERVISOR: Manager of Case Management
POSITIONS SUPERVISED: Caseload Participants
GENERAL DUTIES: To provide case management, quality programming, and supervision to caseload participants. Experience with medically fragile individuals is a plus.

SPECIFIC DUTIES:
- To manage a caseload of participants as determined by the Lead Case Manager, and as set forth in the Case Manager's Manual.
- To maintain an accurate and current electronic case record for each caseload participant.
- To serve as an agency and community resource to each caseload participant, and to make appropriate referrals to other RCS services, funding case managers, and other community services.
- To supervise and coordinate the implementation of each caseload participant's Individual Program Plan.
- To provide counseling, guidance, and instruction as necessary for each caseload participant's development.
- To communicate relevant and significant issues regarding each caseload participant to the President, Human Services Manager and Human Services Operations Manager and the following as appropriate: participant's guardian, family, residential provider, funding case manager, and staff.
- To advocate for each caseload participant's rights, privileges, and responsibilities as specified under Wisconsin Statute sec. 51.61(1), Wisconsin Administrative Code HSS 94, and per RCS policies and procedures.
- To assist in the review and revision of the program’s philosophy, services, and goals.
- To assume additional duties as directed by the Lead Case Manager.
