We are expanding our client management team and seeking a dynamic and experienced Community Liaison to build meaningful relationships with healthcare referrers, educate the community, and help families navigate their care options.
Key Responsibilities:
- Identify and cultivate relationships with local referral sources, including hospitals, nursing homes, doctors’ offices, and other healthcare providers.
- Maintain a database of potential referral partners.
- Conduct educational sessions for professionals and families on dementia and Parkinson’s disease care.
- Schedule and deliver presentations about Reach Angels Agency services at healthcare facilities, private organizations, and trade events.
- Attend trade shows, conferences, and networking events to represent Reach Angels Agency and generate leads.
- Conduct client assessments, develop care plans, and coordinate with office staff to ensure timely care initiation.
- Manage and update the CRM system to track and follow up on leads.
- Report weekly activity and progress to the manager.
- Participate in on-call duties as needed.
Qualifications:
- 2 years of experience in healthcare marketing or community education is highly preferred
- Preferred: LMSW, LBSW, or other relevant healthcare certifications (Occupational Therapist, Physical Therapist, LPN, or RN considered)
- Strong presentation skills and the ability to conduct educational training sessions
- Highly organized with experience in PowerPoint and CRM systems
Candidates must provide measurable results from previous roles, such as:
- The number of new clients signed per month due to their marketing efforts.
- The number of referral partnerships they secured and maintained.
- The conversion rate of leads to paying clients they achieved.
- Any specific strategies they implemented that led to increased revenue or brand awareness.
Compensation:
- Salary Range: $45,000 to $65,000 per year (depending on experience and qualifications)
