Key Responsibilities:
- Coordinate and facilitate the development of individualized care plans and associated documentation.
- Schedule regular meetings to review and update plans, ensuring timely interventions and support.
- Advocate for and make referrals for services based on assessed patient needs, promoting holistic well-being.
- Monitor patient participation and progress, conducting regular visitation reviews to inform care decisions.
- Ensure all duties are performed in accordance with facility policies and guidelines, prioritizing quality and safety.
Qualifications & Requirements:
- Bachelors Degree in a human services field required, with a preference for Masters degree holders.
- (3) years applicable experience in mental health settings is preferred, demonstrating expertise and passion for patient-centered care.
