How you make a difference

As a compassionate Reentry Specialist, you will play a crucial role in preparing individuals for discharge, ensuring continuity of care, and reducing recidivism rates among program completers.

Key Responsibilities

  • Coordination of Aftercare and Referrals: Act as the primary contact for community agencies, schedule follow-up appointments, and ensure seamless aftercare planning for individuals transitioning to the community.
  • Access to Medications and Benefits: Assess needs for psychotropic medications and benefits, assist with applications, and collaborate with treating psychiatrists for medication adjustments, if needed.
  • Housing and Legal Requirements: Evaluate housing needs, provide referrals, and ensure documentation and follow-up on legal requirements such as court dates and mandated services.
  • Continuity of Care: Maintain connections with the facility of origin and the discharged individual, providing follow-up care and support to ensure a smooth transition.
  • Culturally Competent Treatment: Deliver individualized and group interventions with age-specific and culturally competent care, empowering individuals for long-term success post-discharge.

Qualifications & Requirements

Education:

  • Required: Bachelor’s degree in social work, counseling, or related field.

Experience:

  • Preferred: Two (2) years of experience.
