Job Summary
We are seeking an experienced Clinical Social Worker with a Master's degree from an accredited school of social work. The ideal candidate will have 1-2 years of post-graduate experience in a healthcare setting, preferably in a hospital or home care environment.
Responsibilities
- Assist patients and families in meeting their psychosocial needs, promoting overall well-being and quality of life.
- Collaborate with healthcare teams to develop comprehensive care plans that address the physical, emotional, and social aspects of patient care.
- Conduct thorough assessments, identifying areas of concern and developing strategies for improvement.
- Provide education and guidance to patients and families on managing chronic conditions, navigating the healthcare system, and accessing community resources.
Requirements
- Master's degree from an accredited school of social work.
- Eligibility for licensure in New Jersey and/or Pennsylvania.
- 1-2 years of post-graduate experience in a healthcare setting (hospital/home care preferred).
- Strong computer skills and knowledge required.
- Flexibility, sound judgment, customer orientation, organization, and professional demeanor essential.
