What are the responsibilities and job description for the LVN position at Redlands Community Hospital?
The Medication Assisted Treatment Coordinator (MAT) guides patients and their families following the patient's discharge from the hospital to facilitate a seamless transition home and appropriate follow-through and monitoring of the care plan while the patient learns to self-navigate the health care system. The position is integral in supporting the treatment model of medication for addiction treatment in the acute care setting. The y help reduce worry and frustration for patients, families, and caregivers and decrease high rates of hospital readmittance by increasing communication, coordination, and efficiency across the healthcare system; performs various activities including, but not limited to: direct discharged patients to appropriate follow-up care and assist with scheduling appointments; facilitate transitions from the hospital to outpatient treatment, obtain referrals or authorizations; arrange transportation; contact patients prior to appointments as needed to ensure follow-through; document activities of patient advocacy; develop relationships with Primary Care Physicians and Specialists to facilitate a seamless transition and expedite necessary medical appointments; maintain knowledge of and collaborate with local healthcare-related resources; coordinate care with home health agencies and durable medical equipment companies; work with pharmacies for coverage of medication, prescription pickup/delivery/discounts; and keep patients, their families, and caregivers apprised of care management. The MAT will be required to attend California Bridge Substance Use Navigator Training and demonstrate the skills learned to continue employment in this position .

EDUCATION/TRAINING/ EXPERIENCE:
- High school diploma or GED required
- Experience in customer services required.
- Healthcare experience required
- Caregiver experience preferred
- Experience working with individuals within hospitals or public health setting preferred
- Experience with underserved populations preferred
CERTIFICATIONS/LICENSES:
- California Bridge Substance Use Navigator Training required .
