The Peer Navigator will bridge and support case management services to help peers / clients (others with lived experience of homelessness) flourish. A Peer Navigator is a person who has experienced homelessness and is ready to share their expertise in supporting others to transition out of homelessness.
The Peer Navigator will :
Serve as a formal Peer Leader in the operations of street outreach, serving as a peer mentor in sharing their lived experience as comfortable and in a supportive manner
Assist the Supportive Services team with accurate record keeping
Support Street Outreach Team program development initiatives
Guide peers in homeless systems navigation
Continuously work to build and maintain trusting relationships with peers while working alongside the Street Outreach and Supportive Services teams
Support peers in goal attainment, including but not limited to : effective transition to housing, paths to employment, and health management
Ensure the outcomes listed by the U.S. Housing and Urban Development for the Street Outreach Program are being met
The Peer Navigator reports to the Director of Programs.
Position Responsibilities
Program Delivery (85%)
Encourage and provide peer support to clients in Street Outreach and Engagement Center when clients visit :
As a person with similar life experiences, sharing and pulling from personal experiences to support peers with problem-solving and working towards their own goals
Support clients to :
Navigate homeless systems by planning and accompanying clients to necessary programs and services based on support need (in pairs with other staff)
Develop skills to effectively transition from homelessness to housing
Develop self-advocacy skills and related knowledge
Access healthcare, housing and related community resources and services
Work with clients and their supportive services team to develop and document their goals
Use motivational interviewing, trauma informed care, and harm reduction strategies to support clients toward housing stability, increased skills and income, improved health, and achievement of personal goals
Assist in the services for people experiencing unsheltered homelessness visiting the Engagement Center, including but not limited to welcoming clients, organizing and running peer groups, coordinating resource access, and facilitating relationships among peers
Support verbal de-escalation in crisis intervention as able in accordance with agency policy
Organize and distribute transportation assistance, food assistance, and donated goods in accordance with agency policy
Maintain updated knowledge of community resources to share with clients
Perform other duties as assigned
Program Administration (15%)
Track and document all client contacts and all services provided timely and accurately in agency’s system database
Assist Supportive Services staff in data tracking and record keeping
Maintain confidentiality of all client files
Attend regular trainings and capacity building programming to employ best practices
Provide formal feedback on organizational programming
Work collaboratively with Revive staff to identify and develop new position responsibilities
Qualifications
Lived experience of homelessness, homeless and housing systems navigation
Effective transition to housing with sustained housing for at least 6 months
Graduated from ReVive’s housing and homeless service programming, if applicable, for at least 1 year
Skills and experience related to peer support, social service, mental health and substance use
Strong communication, social interaction and interpersonal skills
Able to work effectively with persons who may be ill, disabled, emotionally dysregulated and / or agitated through crisis intervention and de-escalation techniques
Able to show empathy and remain nonjudgmental
Professional demeanor in communication to all people across the organization
Cultural responsivity with respect for diversity (e.g., race, ethnicity, age, gender, sexual orientation, housing status, disability, religion, spiritual beliefs, etc.)
Able to work flexible hours. Evening and weekend work may be required
Bilingual in Spanish and English a plus
Able to work with other team members to make decisions and escalate needs to appropriate support staff
Able to maintain confidentiality of participant information
Able to use technology with effective accommodations to document, communicate through approved platforms and track hours worked
Valid Driver’s License and Auto Insurance a plus
Commitment to the mission of ReVive Center
Upon hire, must complete full ReVive training and onboarding before providing direct service
Must adhere to PN Code of Ethics after training and during first 90-day employment period to continue in role
Compensation
Up to 10 hours per week
Competitive hourly rate paid twice a month
403B savings plan with a 3% agency match
Working Conditions and Physical Requirements
Travel between client and assigned location(s)
Work is normally performed on the street or in the engagement center with some time in an office or client residences with fellow staff member
Exposure to all varieties of Chicago weather
Physical ability to travel to encampments and other locations where Street Outreach occurs
Functional hearing, vision, manual dexterity and hand-eye coordination or effective accommodations in place to perform job functions
Effectively work on teams with interprofessional collaboration
