Role:
The Admissions Coordinator communicates and assesses patients in their place of residency to ensure a smooth transition of care to our Care Team. This person assumes the responsibility of the coordination of an effective and efficient verification process
Job Responsibilities:
- Handles and communicates initial inquiries for care with prospective patients
- Coordinates verification process for all referrals
- Facilitates patient referrals overall
- Completes assessments and admissions of care in prospective patients' place of residency:
- explains services
- addresses and educates on any and all questions
- completes necessary documentation to meet legal and regulatory requirements
- completes documentation according to company policy
- develops plan of care for non-clinical or non-skilled patient assessments/admission
- coordinates needs to nursing team with any and all skilled assessments/admissions
- Acts as company representative in all workplace and remote settings
- Communicates effectively with nursing team to develop full patient assessment
- Develops and fosters relationships with potential referral sources
- Other duties as assigned by leadership team
