Position Summary:
The Housing Coordinator supports individuals with disabilities and mental health conditions in achieving and maintaining stable housing. This role involves assessing clients' needs, creating tailored support plans, and connecting clients with community resources to address housing barriers.
Minimum Qualifications:
- Experience: At least 1 year of experience providing Housing Stabilization Services (HSS) as a housing coordinator or offering social services to individuals with disabilities.
- References: Willingness to provide professional references from a current or previous manager for a mandatory reference check prior to hiring.
- Location: Must be based in Nicollet/Blue Earth County or surrounding area, with the ability to travel and meet clients in the community regularly.
- Technical Skills: Proficiency in Adobe, Microsoft Office, EHR platforms, etc.
- Transportation: Must have a reliable vehicle with insurance, a valid driver’s license, and a good driving history.
- Background Check: Must pass a Minnesota Criminal Background Check and DHS Background Check
- Personal Attributes: Adaptable, effective communicator, able to follow company standards and protocols, capable of working independently, and efficient in time management.
Desired Qualifications:
- Education: Degree in a social service or human services-related field preferred or at least 4 years of experience in the social service field.
- Experience: Providing direct social services.
- Knowledge: Familiarity with local community and housing resources.
- Skills: Experience assisting individuals with mental and/or physical disabilities and socioeconomic barriers.
Essential Duties and Responsibilities:
- Client Management: Handle a caseload of clients who are searching for housing.
- Remote and In-Person Work: Conduct 50% of work remotely and meet clients in person at least once a month, monthly case consultation.
- Case Documentation: Maintain professional case notes daily using time keeping software.
- Barrier Identification: Identify and address barriers to housing access for clients.
- Housing Assessments and Support Plans: Conduct housing assessments and develop support plans to address clients' housing needs and barriers.
- Housing Searches: Perform housing searches and build relationships with landlords, property managers, and owners.
- Client Monitoring: Conduct in-person visits and remote sessions to monitor client progress and provide ongoing support.
- Resource Familiarity: Stay informed about community, county, and state resources for additional client income assistance.
- Benefit Assistance: Assist clients in applying for benefits to help them afford housing.
- Professional Communication: Communicate effectively with case managers, providers, family members, and other stakeholders from diverse backgrounds.
- Program Navigation: Navigate Minnesota Healthcare programs and processes.
- Compliance: Maintain client files and program records in compliance with HIPAA and Data Privacy Practices.
- Team Collaboration: Collaborate with the management team, providing feedback and suggestions for improvement.
